Painful periods, also known as dysmenorrhoea, are one of the most common gynaecological conditions among young women. There are two main types of dysmenorrhoea: primary and secondary.
What is primary dysmenorrhoea?
Primary dysmenorrhoea is the most common form of period pain. The pain is mainly in the lower part of the abdomen but can also radiate into the back and down the front of the thighs.
What is secondary dysmenorrhoea?
Secondary dysmenorrhoea is different. It can develop later, usually in your mid-twenties or even later.
The pain might not just occur during your period but can happen throughout your cycle. Your periods may also become heavier and last longer.
Secondary dysmenorrhoea can be a sign of underlying conditions like endometriosis (the most common cause), adenomyosis, fibroids, or ovarian cysts.
What are the causes of period pain?
The pain you feel during your period comes from the uterus contracting to shed its lining. Along with these contractions, there is also a reduction in blood flow to the uterus, which adds to the discomfort.
The hormone prostaglandin plays an important role in this process. Prostaglandins are produced in the lining of the uterus, and on the first day of your period, their levels are high. This is why the pain tends to be more intense at the start. As the lining is shed, the levels of prostaglandins decrease, which is why the pain usually starts to lessen as your period goes on.
How do painkillers reduce the pain?
Many women get bad cramps during their period. The reason may be due to a hormone imbalance.
Taking a painkiller will stop the pain by stopping the release of the hormone, prostaglandin. When this hormone is not released, the brain will not get the message as quickly or clearly.
The medication stops you feeling the symptom but it does not address the cause of the pain.
